CKD

5/6 nephrectomy

Surgical reduction of renal mass driving hyperfiltration, hypertension and progressive glomerulosclerosis.

Background and rationale

Removal of approximately five sixths of functional renal mass leaves the remnant kidney hyperfiltrating, which over weeks produces hypertension, proteinuria, glomerulosclerosis and interstitial fibrosis. It remains the classic model of progressive CKD and of cardiorenal and mineral bone complications.

Typical study design

  • One- or two-stage surgery (polar resection or ligation) with sham controls
  • Baseline stratification by proteinuria and blood pressure before randomisation
  • Non-invasive blood pressure monitoring (CODA tail cuff)
  • Long-term dosing with interim functional readouts

Readouts

  • Plasma creatinine, BUN, cystatin C, phosphate, PTH and FGF23
  • Urinary albumin-creatinine ratio and protein excretion
  • Systolic blood pressure by CODA tail cuff
  • Glomerulosclerosis and fibrosis scoring with AI-assisted histology
  • Cardiac assessment by ultrasound
  • RNA sequencing and spatial transcriptomics
